LONG TERM CONDITIONS & MEDICATION REVIEWS
Our patients that have a long term condition or are on a high risk medication will be monitored and reviewed within the Practice and called for according to your birth month. Depending on what your long term condition is or what medication you are taking will depend on what tests are required. Below is a table that identifies when you can expect correspondance from us;

If you are a new patient or haven't heard from us after referring to the above table then please get in touch with Molly or Carla and we will arrange the necessary tests.
The blood tests are requested by the surgery and sent electronically to Rowley Regis Hospital. You will need to contact them on 0121 507 6104 or swbh.phlebotomy@nhs.net to book an appointment (please state your name, date of birth, and NHS number with your preferred date and time). If requested for a urine test you can collect a sample bottle and forms from Reception. These should be returned to the surgery any weekday before 12pm to enable the hospital courier to collect and return to the Hospital for analysis.
Once these tests have been booked please book a face to face appointment with our Health Care Assistant, Ina or Karen, for a physical health check. They will then book you a telephone medication review with a Clinician to discuss the results from your blood test, create a care plan and reauthorise any repeat medication.
POST NATAL, BABY & PRE-SCHOOL IMMUNISATIONS
The surgery will contact Mum and baby at 8 weeks for a review, health check and babies 1st immunisations. These are carried out by your Doctor and Nurse.
Future immunisations will be called for through the local Child Health programme. Appointments will be held here with our Practice Nurse. It is important that your child's 'red book' is brought to every appointment as this will hold vital information that you will need when your child starts school.
If you are unable to attend the scheduled appointment then please contact Reception who will gladly re-book for a convenient day/time.
IF you have any concerns about specific vaccines please call in to speak to the nurse or one of the GPs.
LEARNING DISABILITY HEALTH CHECKS
Our patients are recalled by the surgery for an annual health check according to your birth month.

Haden Vale Medical Practice has recently been awarded 'GOLD' for our achievement with our Learning Disability patients from Sandwell Learning Disability Community Specialised Health Service.
WELL WOMAN CLINIC & SMEARS
Screening is offered to all women aged 24.5years and 64years. Women aged 24.5years will received their first invitation by post to ensure they can be screened before they are aged 25 years.
25years - 49years should be offered screening every 3 years.
50years - 64years should be offered screening every 5 years.
If a previous result was abnormal you may be called in sooner.

As well as looking after our patients with a long term condition we are here to advise all our patients on a variety of health related problems:

FAMILY PLANNING
Sara our ANP can offer contraceptive advice and pill prescriptions etc.
MENOPAUSE/HRT CLINIC

TRAVEL VACCINATIONS
Please fill in a Travel Health Questionnaire (minimum 6 Weeks) prior to travelling abroad for the nurse to assess your travel advice and vaccination requirements. Reception staff will book an appointment once your for as been checked with the nurse.

MINOR SURGERY
Joint Injections
Dr. Muthuveloe carries out joint injections. If you feel that you may benefit from this service then please book a routine telephone consultation with any of our Doctors and they will add you to our waiting list.
These appointments are usually carried out on a Thursday afternoon.
PHLEBOTOMY
This service is now provided at Rowley Regis Hospital, Moor Lane, Rowley Regis
West Midlands. B65 8DA. We can send your blood test forms directly to this department. To book an appointment you can call them on 0121 507 6104 or email swbh.phlebotomy@nhs.net (please state your name, date of birth, and NHS number with your preferred date and time).
PRIVATE MEDICAL REPORTS/REQUESTS
We can provide a range of non NHS services such as pre-employment medicals for HGV, PCV, taxi etc. When booking the appointment, it is important that you explain to the receptionist that it is for such an examination, as more time is needed than for a routine appointment
